Membership and medical information form To become a member of Ware
Swimming Club, swimmers will be required to attend a trial night so that they
can demonstrate to the club official that they are able to swim three out of
four strokes. Upon completion of
a successful trial, swimmers will be invited to apply to join Ware Swimming
Club. To apply for membership
swimmers should complete and sign the following documents: Application Form Club Rules Code of Conduct ASA Registration Form
(Single or Multi Club) Please
note, failure to complete the correct membership form could result in
